Insurance |
Although our office will file your primary insurance as a courtesy, we do need to have your insurance card. To ensure all information is correctly processed for you, we need a physical card at each visit. We do not file any secondary insurance. A copy of your insurance card and a photo id will be made at the time of each visit and also at least once a year, before you see the doctor. Our office will verify your insurance card and general information at each visit. Plans we participate with:

Our office is open: Monday through Friday, 8:30 am – 5:00 pm. Lunch time appointments are available. The office is closed weekends and most holidays. |
